[ castiel, of course, simply takes it at face value, because he has no reason not to. why wouldn't sam order the pizza with his brother in mind, if he knows dean's here? that's just simple consideration. that's not to say that castiel has a lot of experience; he's not.. particularly close with his brothers, they certainly wouldn't think to consider him when figuring out food for themselves, but castiel's self-aware enough to know that his situation isn't the standard model. that there are plenty of people who love their siblings very much, wouldn't think twice about grabbing a pizza for their brother.
and all right, he doesn't really know much about sam and dean's relationship beyond what sam's mentioned, and he's certainly done a lot of bitching about dean, but it's always been innocent enough. normal enough. castiel hasn't really pressed for more.
